Job Description

We seek a Case Manager to provide comprehensive case management services for clients experiencing crises, including mental health challenges, substance abuse issues, and intellectual disabilities, ensuring they receive the necessary interventions, referrals, and long-term support to improve their well-being. The role includes navigating complex social service systems, including the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM-CANS) and Social Security Disability Insurance (SSDI). The Case Manager is critical in guiding clients through application processes, advocating for their needs, coordinating care with external agencies, and facilitating appropriate crisis interventions. This position involves assessments, treatment planning, advocacy, transportation coordination, and emergency intervention for individuals at risk of hospitalization.

This role is onsite at our Bucktown Clinic, 2409 N Clybourn Ave, Chicago, IL 60614, Monday-Thursday, 9 am to 5 pm.

Key Responsibilities:

Client Assessment & Planning:

• Conduct comprehensive assessments to evaluate client needs, health status, financial situations, and eligibility for various services.
• Develop individualized treatment and action plans to address clients’ social, financial, health, and legal needs.
• Perform crisis risk assessments and develop appropriate crisis intervention plans.

Case Management & Coordination:

• Guide clients through IM-CANS, SSDI, ABE, Medicaid, and other social service applications, ensuring they complete all required documentation accurately.
• Act as a liaison between clients and external agencies, advocating on their behalf for financial, healthcare, and community-based resources.
• Maintain consistent communication with clients to provide updates on application status and service availability.
• Coordinate discharge planning with healthcare providers, third-party payors, and other support agencies to ensure continuity of care.
• Arrange and provide transportation for clients to essential services and discharge destinations.

Crisis Intervention & Emergency Services:

• Provide crisis intervention services in both community and residential settings.
• Assist in de-escalating clients experiencing acute mental health crises.
• Refer individuals in crisis to appropriate resources, including crisis residential units, emergency psychiatric care, and social services.
• Participate in an interdisciplinary team to evaluate and coordinate appropriate client care.

Documentation & Compliance:

• Maintain detailed, confidential, and accurate case files, documenting client interactions, crisis assessments, action plans, and follow-ups.
• Ensure compliance with company policies, state regulations, and legal documentation requirements.
• Review and update clients’ treatment and action plans regularly to reflect changes in circumstances or needs.

Collaboration & Team Support:

• Collaborate with case managers, healthcare professionals, and social service agencies to provide holistic, high-quality care for clients.
• Support team members in reviewing applications and addressing documentation and service coordination challenges.
• Participate in team meetings, offering strategies and feedback for improving case management and crisis intervention outcomes.

Qualifications:

Education:

• Bachelor’s degree in social work, healthcare, human services, psychology, or a related field preferred.

Experience:

• Minimum of 2 years of experience in case management, crisis intervention, healthcare, or social services.
• Experience with IM-CANS, SSDI applications, Medicaid/Medicare processes, and crisis response services is highly desirable.
• One year of experience working with individuals with mental illness, intellectual disabilities, and/or substance abuse issues.

Skills & Abilities:

• Strong organizational skills and attention to detail.
• Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.
• Ability to handle sensitive and confidential information with professionalism and discretion.
• Proficiency in Microsoft Office Suite and general office software; experience with case management systems is a plus.
• Ability to work independently and manage multiple tasks in a dynamic environment.
• Bilingual in Spanish/Arabic preferred.

Work Environment and Physical Requirements:

• Comfortable working in both a clinic setting and in the community.
• Must be adaptable and comfortable working with diverse populations.
• Strong problem-solving skills and the ability to navigate complex healthcare and social service systems.
• Ability to lift up to 15 pounds occasionally and work at a desk for prolonged periods.
