We seek a Case Manager to provide comprehensive case management services for clients experiencing crises, including mental health challenges, substance abuse issues, and intellectual disabilities, ensuring they receive the necessary interventions, referrals, and long-term support to improve their well-being. The role includes navigating complex social service systems, including the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM-CANS) and Social Security Disability Insurance (SSDI). The Case Manager is critical in guiding clients through application processes, advocating for their needs, coordinating care with external agencies, and facilitating appropriate crisis interventions. This position involves assessments, treatment planning, advocacy, transportation coordination, and emergency intervention for individuals at risk of hospitalization.
This role is onsite at our Bucktown Clinic, 2409 N Clybourn Ave, Chicago, IL 60614, Monday-Thursday, 9 am to 5 pm.
