Licensed Master Social Worker – Nursing Home Surveyor/Complaint Investigator – Central Islip, NY (#25199)

$39/hr | Flexible Schedule | Travel/Field-Based Role

What We’re Looking For:

• NYS Licensed Master Social Worker (LMSW)
• Minimum 2 years of clinical experience in a medical setting (nursing home preferred)
• Prior experience in nursing home surveillance is a plus
• SMQT Certification preferred (or willingness to obtain within 12 months)
• Strong written communication skills and the ability to travel locally for on-site investigations

What You'll Do:

• Conduct onsite surveys and complaint investigations involving issues such as abuse, neglect, and care quality
• Draft Statements of Deficiencies (SOD) using established regulatory principles
• Evaluate Plans of Correction submitted by facilities
• Enter critical findings into federal/state databases
• Participate in state-level monitoring , especially during urgent or “Immediate Jeopardy” scenarios
• Provide testimony in administrative hearings when needed
• Attend regular team briefings and maintain compliance with DOH and IPRO protocols
