Job Description

Medix is currently hiring for a Social Worker Care Coordinator at a University in New Haven CT. We are open to either LMSW or LCSW. The schedule is M-F: 9am-5pm - 37.5 hours a week.

Job Description

The Care Coordinator is responsible for coordinating clinical care and managing referrals as assigned. This role ensures that students are promptly connected to a clinician and takes the lead in managing the care of patients with complex needs. The Care Coordinator serves as the primary point of contact for students, facilitating access to both internal and external mental health resources.

Overview

  • Manages clinical care for students, ensuring timely access to services and follow-up support.
  • Communicates with patients, families, clinicians, and staff regarding treatment and program implementation.
  • Coordinates referrals between mental health counselors, academic departments, ensuring proper fit.
  • Assesses students' clinical needs and collaborates on appropriate care plans, maintaining patient records.
  • Provides brief assessments and interventions while students connect with providers.
  • Coordinates external referrals to partners with senior staff
  • Develops and maintains tracking systems for referrals, feedback, and utilization metrics.
  • Maintains a database of local, national, and international mental health resources.
  • Authorizes referrals and collaborates with Member Services, Claims, and Billing for service coverage.
  • Acts as the primary contact for students and clinicians regarding external services and placement.
  • Supports students through the referral process, ensuring timely scheduling with community providers.
  • Builds strong relationships with students, clinicians, and stakeholders.
  • Adheres to privacy and confidentiality standards for health information.
  • Provides educational resources and consultations to various departments.

3-5 Must Have Skills/Qualifications

  • State of CT Clinical Social Worker License or CT Clinical Social Worker license eligible.
  • Master’s Degree in Social Work and three years providing clinical care post licensure.
  • Experience in care management (clinical)
