Job Title: Inpatient Social Worker (BSW or Higher) – 13-Week Contract
Location: Portsmouth, NH
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Job Type: Contract (13 Weeks)
Pay: Competitive | Guaranteed 40 hours/week
Job Summary:
We are seeking an experienced and compassionate Inpatient Social Worker to join a reputable hospital in Portsmouth, NH for a 13-week contract assignment. The ideal candidate will have a strong background in hospital-based social work, including discharge planning, guardianship, and long-term care coordination.
Key Responsibilities:
Manage inpatient case management and discharge planning
Work with families on guardianship, surrogacy, and long-term care placement
Document using Expanse and CarePort systems
Collaborate with a multidisciplinary team of Social Workers and RN Case Managers
Float to other assignments as needed within the hospital
Requirements: -
Minimum 2 years of experience in a hospital setting
BSW or higher (required)
Current BLS Certification
Strong interpersonal and communication skills
Ability to work independently in a fast-paced environment
Additional Details:
No weekends
