About the Role...

Job Details/Quick Notes:

• Profession: Licensed Clinical Social Worker (LCSW) - Social Services
• Location: Baltimore, MD
• Start Date: 5/19
• End Date: 8/18
• Length: 13 weeks with extension opportunity or perm!
• Schedule: Monday - Friday; NO major Holidays, and NO on-call
• Shift: 5x8's Days

Travel vs. Local:

• Travel Contract which requires the LCSW to live 51 miles outside of Baltimore, MD to qualify for Stipend
• Will Accepted a Local Traveler within 60 miles inside Baltimore, MD will not qualify for Stipend.

Travel Pay Package:

• Untaxed Travel Stipend: $1,652
• Weekly Gross: $2,150 - $2,250

Local Pay Package:

• Taxed Hourly: $45 - $50 depending on experience.

Skill Required:

• Case Management, Medical Surgical Experience, Admission Criteria, Care coordination, Discharge Planning, Plan of Care, Assess response to treatment, Biopsychosocial assessment, Collect and analyze patient information, Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs, Development of problem list, Establish and revise goals, Review of Health history, Collaborative care, Coordinate services to home care and equipment following discharge, Coordinates quality care in cost effective manner, Refers to rehab and long term care facilities, Documentation according to plan of care, Documentation of care delivery, plan, goals, Documentation reflects understanding of coverage criteria, Documents patient response, Facilitate discharge and contingency planning, Long-range planning and decision making, Medicaid, Medicare, Adult protective services, Child protective services, Advance Directives, Skills Preferred: LTC/Nursing Homes, Benefits Eligibility, Data Abstraction, Determine Medical Necessity per Evidence-Based Guidelines, Disease management, Needs Assessment/ Order DME, Prior Authorizations, Documentation of patient/caregiver teaching/learning, Provide crisis intervention

Required Experience/Licensure:

• LCSW, MD STATE LICENSE
• No On Call/ Weekend Requirements
• Float Required to Med Surg, Med Surg Tele, Tele, ICU-CCU, and Observation Units
• First Timers Accepted: Yes (Only If candidate has relevant/current hospital Case Management experience on med/surg unit)
• Patient Ratio Experience Required: 1:18
