Position Overview

Greenlife Healthcare Staffing is seeking a Licensed Master Social Worker (LMSW) to work as a Nursing Home Surveyor/Complaint Investigator for a reputable Review Company based in New York, NY . This role is ideal for an LMSW with clinical and regulatory experience who is passionate about ensuring quality care in long-term care settings.

Qualifications

• Licensed Master Social Worker (LMSW) with active New York State license
• Minimum 2 years of clinical experience in a medical facility (nursing home preferred)
• Preferred: 2 years of nursing home surveillance or complaint investigation experience
• SMQT Certification preferred (must be willing to obtain within 12 months)
• Valid driver’s license and ability to travel to assigned onsite facilities

Responsibilities

• Conduct surveys and complaint investigations in long-term care facilities, including abuse or neglect allegations
• Perform surveillance activities to assess compliance with Federal and State regulations
• Draft and submit Statements of Deficiencies (SODs) using the Principles of Documentation
• Review and evaluate compliance with Plans of Correction
• Enter findings into Federal and State databases
• Participate in State monitoring during Immediate Jeopardy situations
• Attend meetings and testify at administrative hearings as required
• Adhere to DOH policies , procedures, and schedules
• Travel to onsite locations as scheduled
